- Download the Registration Form
(pdf) and fax it to the number on the form.
- Call Registration Services at 435-797-0423 or toll free 800-538-2663 with your credit card
information.
CART and Sign Language Interpretation Services are available
if scheduled in advance. Please indicate when you register if you will need such
services. CART and ASL services will be provided free of charge for requests made prior to February 4, 2008.
